*Monthly memberships require a 12-month commitment. Annual memberships offer the greatest value.
*Important Membership Information
Membership fees do not include costs associated with outside laboratories, imaging centers, hospitals, specialists, medications, or other third-party healthcare services. Patients are encouraged to maintain active health insurance coverage for medical services performed outside of the clinic.
For the Hybrid Concierge Membership
1. Membership is NOT insurance
• Membership fees are not insurance premiums.
• Membership does not satisfy any insurance requirements.
• Membership does not cover hospitalization, emergency care, specialist care, imaging, or outside services.
2. Insurance will still be billed
• Covered services may be billed to insurance.
• Deductibles, copays, coinsurance, and non-covered services remain the patient’s responsibility.
