Coronary artery calcium score review when applicable
Review of cardiovascular risk markers
Medication management related to cardiovascular risk factors
Lifestyle counseling
Coordination of appropriate cardiovascular testing
Referral to and collaboration with a cardiologist when specialty cardiovascular evaluation or management is appropriate
The IKANDY Method" is not a cardiology specialty practice. When specialist cardiovascular care is appropriate, referral to and/or collaboration with a cardiologist will be recommended.
CHRONIC CONDITION MANAGEMENT
Ongoing management of appropriate primary care conditions may include:
Hypertension
Hyperlipidemia
Prediabetes and diabetes
Weight and metabolic concerns

Other chronic medical conditions appropriate for primary care management
Treatment may include medication management, laboratory monitoring, lifestyle recommendations, diagnostic evaluation, and specialist referral when appropriate.
ACUTE PRIMARY CARE
Because Hybrid Concierge members establish a comprehensive primary care relationship with The IKANDY Method™, appropriate acute medical concerns may be evaluated and managed as part of their primary care.
The need for an office visit, telehealth visit, urgent care evaluation, emergency evaluation, or specialist referral will be determined based upon the patient's individual clinical circumstances.
MEDICATION MANAGEMENT
Membership includes comprehensive medication oversight as part of the patient's primary care relationship, including:
Medication reconciliation
Medication monitoring
Review of potential adverse effects
Appropriate prescription management
Treatment adjustments when clinically indicated
Coordination with specialists regarding medications when appropriate
Prescription medication costs are not included in the membership fee.
LABORATORY & DIAGNOSTIC COORDINATION
The practice may order and coordinate clinically appropriate laboratory and diagnostic testing.
This may include:
Routine laboratory testing
Preventive laboratory testing
Cardiometabolic testing
Lipid and cardiovascular risk markers
Hormone-related testing when appropriate
Weight-management monitoring
Imaging
Cardiovascular testing
Other clinically indicated diagnostic studies
Laboratory, imaging, diagnostic facility, and third-party charges are not included in the annual membership fee unless specifically stated otherwise.

Assistance with continuity between multiple healthcare providers
Specialist services are not included in the membership fee.
HOSPITAL & EMERGENCY CARE FOLLOW-UP
When a member receives care through an emergency department or hospital, The IKANDY Method™ may assist with appropriate follow-up and continuity of care.
This may include:
Review of available hospital or emergency records
Medication reconciliation
Follow-up planning
Coordination of recommended testing
Specialist coordination
Transition back to outpatient primary care
Hospital and emergency services are not included in the membership fee.
ANNUAL COMPREHENSIVE HEALTH REVIEW

WHAT IS BILLED TO INSURANCE
The $2,000 annual membership fee does not replace the patient's health Insurance.
When applicable, medically necessary covered medical services are submitted to the patient's participating Insurance plan.
Depending upon the patient's Insurance coverage, this may include:
Office visits
Telehealth visits
Preventive services
Chronic disease management
Acute medical evaluation
Medication management
Other medically necessary covered professional services
Insurance coverage cannot be guaranteed.
Patients remain responsible for all applicable Insurance cost-sharing requirements, including copayments, deductibles and coInsurance.

Unless specifically stated otherwise, the annual membership fee does not include:
Insurance copayments

The Hybrid Concierge Primary Care Membership does not provide:
Unlimited medical visits without Insurance billing
Guaranteed same-day appointments
Guaranteed appointments outside normal business hours
24/7 emergency medical services
Unlimited messaging or texting
Hospital or emergency medical care

IMPORTANT INSURANCE & MEMBERSHIP NOTICE
The Hybrid Concierge Primary Care membership fee is separate from charges for medically necessary covered medical services submitted to Insurance.
Membership benefits are intended to provide enhanced access, communication, coordination, convenience, and other specifically defined membership benefits.
Insurance coverage and patient financial responsibility are determined by the patient's individual health plan.
The final membership agreement, Insurance billing structure, Medicare participation requirements, discounts, and designation of covered and non-covered services should comply with applicable payer contracts and federal and state requirements.

Full Concierge Primary Care Membership
$4,000 Per Year No Insurance Billing for Included Primary Care Visits
The Full Concierge Primary Care Membership is designed for patients who would like The IKANDY Method™ to serve as their comprehensive primary care practice through a simplified cash-pay model with unlimited included primary care visits, enhanced access, personalized medical management, and comprehensive care coordination.
The annual membership provides the highest level of access offered through The IKANDY Method™, while allowing members to maintain and independently utilize their health Insurance for outside services such as laboratory testing, imaging, medications, specialists, hospital services, and other eligible healthcare expenses when permitted by their individual health plan.

The membership does not provide 24/7 emergency medical services.
ENHANCED COMMUNICATION
Full Concierge members receive enhanced communication with the practice for appropriate medical questions, medication concerns, treatment-plan questions, results, and care coordination.
Enhanced communication is intended to improve continuity, convenience, and coordination of care.
Some medical concerns require an office or telehealth evaluation and cannot be appropriately managed through messaging alone.
Emergency medical concerns should always be directed to appropriate emergency services.
COMPREHENSIVE PREVENTIVE HEALTH PLANNING

The IKANDY Method™ is not a cardiology specialty practice.
When symptoms, medical history, examination findings, or diagnostic testing indicate that specialty cardiovascular evaluation or treatment is appropriate, referral to and/or collaboration with a cardiologist will be recommended.

Weight and metabolic health assessment
Nutrition and lifestyle counseling
Body-composition assessment when available
Evaluation for prescription weight-management medication
GLP-1/GIP and other appropriate medication management
Medication titration
Side-effect monitoring
Relevant laboratory monitoring
Ongoing weight-management follow-up
Professional primary care visits related to weight management are included within the membership.
Prescription medications, compounded medications, outside laboratory charges, and other separately provided products or services are not included unless specifically stated.
HORMONE HEALTH & MANAGEMENT
Hormone-related medical evaluation and management may be incorporated into the Full Concierge member's comprehensive primary care plan when clinically appropriate.
This may include:
Menopause and perimenopause evaluation
Male and female hormone assessment
Symptom assessment
Review of relevant laboratory testing
Evaluation of treatment indications and contraindications
Discussion of treatment risks, benefits, and alternatives
Prescription hormone management when medically appropriate
Ongoing symptom monitoring
Laboratory monitoring
Medication adjustment
Referral to appropriate specialists when indicated
Prescription medications, compounded medications, specialty laboratory testing, and hormone procedures are priced separately unless specifically included as a membership benefit.
ONE COMPLIMENTARY HORMONE PELLET TREATMENT PER YEAR
One Complete Hormone Pellet Treatment Per Membership Year Full Concierge Primary Care members are eligible for one complete hormone pellet treatment per membership year.
The annual pellet benefit includes:
Clinical assessment related to pellet treatment
One hormone pellet treatment
Hormone pellet product used for that treatment
Pellet placement procedure
Standard procedure-related supplies
The complimentary pellet treatment is available once during each active 12-month membership period when hormone pellet therapy is clinically appropriate.
Hormone pellet therapy is not guaranteed solely because a patient is a Full Concierge member. Treatment remains subject to appropriate medical evaluation, clinical indication, contraindication screening, informed consent, and the clinician's medical judgment.
The annual pellet benefit:
Is limited to one complete pellet treatment per membership year
Is non-transferable
Has no cash value
Cannot be exchanged for another service
Does not roll over into a subsequent membership year if unused
Does not entitle a member to pellet treatment when treatment is not clinically appropriate
Additional pellet treatments during the same membership year are priced separately and may qualify for the Full Concierge member discount when eligible.

USING HEALTH INSURANCE OUTSIDE THE PRACTICE
The IKANDY Method™ does not bill Insurance for primary care visits included within the Full Concierge membership.
Members may maintain health Insurance and may independently use their Insurance for eligible outside healthcare expenses according to the terms of their individual plan.
This may include:
Prescription medications
Laboratory testing
Imaging
Diagnostic testing
Specialist care
Cardiology care
Emergency care
Hospital services
Other covered outside healthcare services
Coverage and patient financial responsibility are determined by the member's individual health plan.
WHAT IS NOT INCLUDED IN THE MEMBERSHIP FEE
Unless specifically identified as an included benefit, the annual membership fee does not include:

Members may maintain health Insurance for prescription medications, laboratory testing, imaging, specialists, hospital care, emergency services, and other outside healthcare expenses according to the terms of their individual health plan.
The annual complimentary hormone pellet treatment is subject to clinical appropriateness, informed consent, and applicable prescribing and treatment requirements.
The final membership agreement, Medicare policies, Insurance arrangements, hormone therapy benefits, discounts, and definitions of included and excluded services should comply with applicable federal and state requirements, payer agreements, and professional scope-of-practice requirements.
